A 72-year-old woman presents with two weeks of intermittent palpitations. She has hypertension and type 2 diabetes mellitus and no history of bleeding, falls or liver disease. Her pulse is irregularly irregular at 88/min and blood pressure is 136/82 mmHg. ECG confirms atrial fibrillation with no other abnormality. Echocardiography shows a structurally normal heart with no mitral stenosis, and eGFR is 68 mL/min/1.73 m².
Which of the following is the most appropriate long-term antithrombotic therapy?
Correct answer: C.
Her CHA2DS2-VASc score is 4 (age 65–74, female sex, hypertension, diabetes); even with the sex-neutral CHA2DS2-VA score used by ESC 2024 it is 3, well above the threshold for oral anticoagulation. In non-valvular AF without moderate-to-severe mitral stenosis or a mechanical valve, a DOAC is preferred over warfarin because it has lower rates of intracranial haemorrhage and needs no INR monitoring. Antiplatelet agents should not be used for stroke prevention in AF. Bleeding risk should be assessed (e.g. ORBIT or HAS-BLED) to modify risk factors, not to withhold anticoagulation.
High-yield: CHA2DS2-VASc ≥2 in men or ≥3 in women (CHA2DS2-VA ≥2) – offer oral anticoagulation. · DOACs are first-line in non-valvular AF; warfarin is required for mechanical valves and moderate–severe mitral stenosis. · Aspirin has no role as sole stroke prevention in AF.

The exam
EMREE is the unified residency entrance examination run by the National Institute for Health Specialties (NIHS) at UAE University. Passing it is one of the requirements for applying to residency training with the UAE health authorities — it is a qualifying exam, not a licence.
It tests clinical application at the level of a graduate with about one year of training, in a UAE context. Results arrive within ten working days and stay valid for up to two years.
